Residency Application

Ready to call Salemtowne home? We are ready to help you start your future. Salemtowne accepts applications from persons age 62 years or older, or in the case of a joint application, one of the two persons must be 62 years or older. Eligible applicants will be considered without regard to their sex, race, religion, color, national origin, political beliefs, or ancestry.

Using the application you can print from this page, or by picking up an application in person complete and indicate your intentions to join our:

  • Waiting List: For applicants who wish to reserve a spot in the future.
    • Complete application
    • Submit $100* for waiting list fee
  • Ready List: For applicants who are ready to move as soon as possible.
    • Complete application
    • Provide copies of your Medicare & Supplementary Insurance Cards
    • Submit $100* for ready list fee

Steps to Move-in

Once an accommodation of your preference is available and you are ready to move you will need to provide documentation of your assets and income, as follows:

  • Property tax statement(s)
  • 1st page of your most recent Federal Income Tax Return
  • Supporting documentation of assets and income (i.e. bank statements, brokerage statements, etc)
  • Documentation of Trust that indicates whether the trust is revocable or irrevocable, if applicable
  • Copies of your Medicare Card and Supplementary Insurance card

Once you have been financially approved you will need to:

  • Submit a completed Physician's Health form
  • Complete a nursing assessment at Salemtowne

Reservation of an accommodation: Once you have gone through the admissions process you can reserve an accommodation, at this time you will:

  • Remit 10% of the entrance fee as a reservation deposit
  • Sign reservation agreement
  • Select your décor choices

Once the accommodation is ready for move-in (approximately 90 days) you will remit the remainder of your entrance fee, confirm a move-in date, and sign the Residence and Services Agreement.
